THE EMPLOYEE WAS OPERATING A WATER TRUCK ON THE 1525 LEVEL. HE BACKED INTO THE 3475 ACCESS TO AN OPEN STOPE AND WENT OVER THE EDGE. HE WAS EJECTED FROM THE VEHICLE DURING THE FALL AND FELL APP ROXIMATELY 75' ONTO THE ROCKS BELOW. EMERGENCY RESPONSE TEAM MEMBERS RESPONDED IMMEDIATELY.
